Cancelling your Appointment
If you are unable to attend an appointment with one of the doctors or nurses, please telephone or use the link at the bottom of this page to cancel your appointment. By giving us as much notice as you can you are helping us to make sure that someone else is given your slot.

Corona Virus

Late Patients
If you are late for your appointment, the reception staff will check with the GP to see if you will still be seen, however if you are more than 10 minutes late you will have to re-book. If you feel you must be seen, please speak to the receptionist who can update the GP.

Patient Registration

We only accept patients who reside within the practice boundary. If you wish to check whether you can register with the practice please contact us on 0345 337 0540.

All patients must be asked to complete a new patient questionnaire, allowing us to provide medical care in the interim period, while your medical records are transferred from your old practice to this one. 

You will need to provide  one form of photo ID eg passport/driving licence and a recent utility bill with your home address listed.

We can accept registration paperwork via e-mail sent to gram.greatwesternadministrator@nhs.scot or you can post by mail the completed forms. We are not accepting paperwork being handed in at the door/reception at this time. You can print off the required forms from the links given below. If you do not have access to a computer or printer please call the Practice on Tel 0345 337 0540 to request that we send paper forms out by post.

Forms required to register:-

For adults and children over the age of 6, you will need to complete 2 forms for each person:

  1. Registration Form (GPR)
  2. New Patient Questionnaire - Adult (there are different forms for adults and children)

For children age 6 and under, you will need to complete 2 forms for each child:

  1. Registration Form (GPR)
  2. New Patient Questionnaire - Child (there are different forms for adults and children)
